Patrick Archambault obtained his medical degree from Université Laval in 2000. He completed postgraduate training to become a member of the Royal College of Physicians and Surgeons of Canada as an emergency medicine specialist at Université Laval in 2005. As a recipient of the dean’s McLaughlin Fellowship bursary, he completed a two-year specialization in critical care medicine at the University of Ottawa and obtained the Royal College of Physicians and Surgeons in Canada certification in December 2007. He also completed in November 2007 a master’s degree in clinical research (emergency medicine) from Université Laval. Dr. Archambault was awarded a Canadian Health Services Research Foundation CADRE postdoctoral fellowship scholarship in 2008.

Since 2008, Patrick Archambault works as an emergency and critical care physician and as a clinician researcher at the CISSS Chaudière-Appalaches (Hôtel-Dieu de Lévis). In 2012, he obtained the FRQS Clinical Research Scholar Junior 1 Award. In June 2016, he was awarded the CIHR Embedded Clinician-Researcher Salary Award. In May 2017, he obtained a project grant from the CIHR for four years and funding from the Département de médecine familiale et de médecine d’urgence at Université Laval, where he is an associate professor.

Overview: The Objective of Dr. Patrick Archambault’s research program is to evaluate the impact of a wiki used as a knowledge translation tool on the quality of healthcare in the emergency department and intensive care unit. He is currently undertaking different projects which integrate this innovative technology based upon the sharing and open access to information. His projects explore the use of a wiki to improve healthcare of different populations, notably elderly people, people facing decisions about end-of-life care and victims of mild traumatic brain injury.

Wikis and collaborative writing applications (CWAs) for knowledge translation

He has dedicated his research career to exploring wikis and other CWAs for knowledge translation in healthcare. Significant contributions include: a) a survey to identify health professionals’ salient beliefs about their intention to use wiki-based knowledge tools to promote best practices in trauma, created during his Canadian Health Services Research Foundation (CIHR) funded fellowship in knowledge translation; b) a CIHR-funded scoping review to explore evidence about the effective, safe, and ethical use of wikis and CWAs in health care; c) a Cochrane review on CWAs in health care and their effects on professional practice and healthcare outcomes. In 2013, he created WikiTrauma, a platform that allows sharing of knowledge tools among health professionals. WikiTrauma is now installed within the CISSS of Chaudière-Appalaches and contains more than 1800 knowledge tools. He also created Wiki101, a theory-based training program developed to address the barriers to using WikiTrauma and to show health professionals how to use WikiTrauma. He is now conducting a mixed-method study assessing the impact of Wiki101 on interprofessional collaboration.

Expertise in shared decision-making (SDM) and patient decision aids (DA)

He is leading three projects on SDM involving the creation of wiki-based patient DAs: a) Funded by the FRQS (2014), his research team adapted an existing DA to help intensive care unit patients decide their goals of care to the Quebec context after a rapid prototyping phase and a user-centred design method. With a catalyst grant from the Canadian Frailty Network (2016), his team is implementing this DA and associated clinician training program we are creating, and evaluating the impact on patients and health professionals. b) With funding from the FRQS (2016) and in collaboration with Erik Hess (Mayo) and Edward Melnick (Yale) he is adapting two existing DA on head computed tomography scans from the United States for adults and children to the Quebec context, after a rapid prototyping phase and a user-centred design method. c) A medical resident under his supervision has adapted a DA on the use of antibiotics for acute respiratory infections to be used in the emergency room, using a rapid prototyping and user-centred design methodology.

Expertise in care transitions for the elderly

In 2015, the CISSS Chaudière-Appalaches was chosen by the Canadian Foundation for Healthcare Improvement (CFHI) as one of 18 sites to improve healthcare for the elderly by adopting and implementing the Acute Care for Elders (ACE) program into the hospital. The multifaceted intervention aims to improve transitions of care for the elderly from the hospital to the community. With a CFHI Catalyst grant (2016), He is leading a process evaluation to understand how the intervention improves health care and can be scaled-up to other hospitals. In May 2017, he obtained Project Scheme Grant from the CIHR for four years. This project will be studying the adaptation of the Acute Care for Elders care transitions best practices developed at the Mount Sinai Hospital in Toronto to the local context of the Centre intégré en santé et services sociaux de Chaudière-Appalaches.
